I have your GTC book, Golden Touch Dice Control Revolution! and the two-disc DVD set on dice control, Golden Touch: Beating Craps by Controlling the Dice. I use a practice rig for my daily practice.
I respect that it's difficult to diagnose without seeing someone actually tossing, but thought perhaps that with your experience you may have an idea on this particular issue I'm having.
Using the Hardway Set as an example [hardway set has 4:4, 2:2, 3:3 and 5:5 set around it, with 6:1 on the ends) the 6 and 1 facing left and right, one die will usually be on axis and more often than not, the out-facing number (6 or 1) will show on the other. Sometimes both.
Is this a common problem with a common remedy?

As you said, it is hard to diagnose a problem without actually seeing the grip, swing and release, but I will give it a try.
Most dice control problems come from the grip. If one die is going off axis and hitting that 1 or 6 side, it is possible that the dice are not starting out flush together and could have a fractional split that makes one go off axis while the other stays on axis. This split could make both go off axis as well. Even the best dice controllers will throw those 1-sides and 6-sides, but if you are doing it with annoying consistency, then check your grip in a mirror before you throw.
It is also possible that one finger on a three-finger grip, either the ring finger or the pointer, is lower than the other. This will cause one die to lag and one die to take the lead. That second die can lose your control over it because there was too much pressure on it when it left your hand.
The third thing to look for is in the swing. if you are bringing your arm into your body as opposed to just bringing it back about six or eight inches, you will be throwing a curve ball or slider — this can also cause one or both dice to go off axis.
